  • Treatment

    Consultations are the foundation of every successful treatment plan. Prior to any treatment you will always have an indepth consultation to assess whether the treatment is suitable for you, or likely to be successful. If not, we will inform you as to the reasons why. Even if you wish to have a particular treatment, if we do not believe that treatment is in your interest, we will decline to treat you.

    There is a fee of €100 for all consultations. A full medical history, skin history and photos will be taken. We will then discuss your concerns, expected outcomes and the treatment options available.

    The treatment plan must be commenced within 3 months of the consultation otherwise a new consultation will be required.

    Please be aware that the outcome of the consultation may be information and advice that you are unsuitable for treatment and you will not be entitled to a refund of your consultation fee.

    Medical history

    It is important that you provide all medical history asked of you. All clients are requested to be aware of their current medical history, any medications they are currently taking and any allergies that they have. It is the responsibility of the client to know these details and to communicate them with their clinician. SK/N D6 will not be held liable for any damage caused because of withheld information or failure to follow pre and post operative instructions from your clinician.

    Consent

    You are required to give written consent to a treatment. You may withdraw that consent at any point or terminate your treatment verbally.

    Children

    • Children and young adults are not allowed in the clinic under any circumstances.

    • We do not treat children or young adults under the age of 21.

    Information

    • All patients are required to provide contact details; including address, telephone and email in order to secure an appointment.

    • All clients will complete a medical history form before or at their first appointment, this is necessary to inform the consultation and treatment planning process.

    • All information will be treated as confidential and protected in accordance with Data Protection legislation.

    • Patient information will not be shared with third parties without written permissions and you will not receive unsolicited information from us.

    • You may choose to remove yourself from our mailing list at any time, by unsubscribing.

    New Patient – Virtual Consultation

    We acknowledge in some circumstances that virtual consultations may be helpful; particularly for patients who will be travelling long distances. New patients seeking advice from a practitioner in a virtual consultation require a diary appointment of 30 minutes. This is booked the same as the in person consultation (select virtual consultation), and the same fee of €100 applies, redeemable against treatment booked / products purchased. This appointment is subject to the usual cancellation and late attendance conditions. Suitability for any treatments discussed will be subject to a physical examination following the virtual consultation.

    Appointments

    • You will be sent appointment reminders 2 days before your appointment, either by text and/or email.

    • If you need to cancel or reschedule your appointment, please contact us in advance of 2 working days (48 hours) before your appointment on hello@skn.ie so that we may make the best use of our appointment diary.

    • Appointments will be subject to a €100 deposit redeemable against products / treatments booked, to mitigate missed appointments, late attendance (15 minutes late or more), or cancellations within the 2 working days preceding the appointment. For appointments with Dr. Sarah Kate, should you fail to attend an appointment or cancel within the time frame , you will lose your deposit.

    • Please do NOT attend the clinic for an appointment if you are unwell. If you are unsure, please call and discuss with the practitioner. Many treatments are contraindicated if you are unwell; this includes colds, cold sores or local skin infections.

    • Routine review appointments are offered after treatment with Botulinum Toxin as a courtesy. No additional treatment or ‘top up’ is provided free of charge, if you fail to attend a review appointment, or once the review period of 2-3 weeks has passed.

    Treatment Courses

    • Treatment courses are valid for 12 months.

    • Treatment courses are non-refundable or transferable.

    • In the event of late or non-attendance, the scheduled treatment will be redeemed from the course to mitigate the loss of clinical time.

    Payment

    • Patients will be charged €100 in advance of the appointment which will be deducted against treatment or product costs. In the event that no treatment is undertaken following the consultation, or patient is deemed unsuitable for treatment, the fee is non-refundable. The deposit will be held on account for 3 months before being redeemed.

    • You will be advised of the full costs of any treatment plan proposed and agreed, including that of any maintenance treatment, before any treatment is undertaken.

    • Payment for treatment is taken, in full at the time of treatment.

    • The clinic accepts cash, or major debit and credit cards.

    • A deposit of €100 will be taken for all treatments.

    Refunds

    Fees charged for treatment are for the delivery of a treatment and the accompanying service, which is inclusive of;

    • consultation and assessment

    • provision of information and advice

    • safe treatment with evidence based products

    • follow up appointments and aftercare advice

    and support as appropriate

    Whilst we undertake to provide excellent service; factual, honest and ethical advice, safe, expert treatment in experienced hands and only the best products, we cannot guarantee your results and cannot offer refunds if the results achieved fail to meet your expectations.

    Although rare, some procedures have well documented complications. Some people may also have higher risk factors, which may contribute to the occurrence of complications. We will, of course, treat these to the best of our abilities and knowledge, as many complications can be managed simply and conservatively. If you require to see Dr. Sarah Kate urgently, you will have to travel to the clinic in Ranelagh for assessment and . or treatment. In the very unlikely event of a complication being more serious and you needing further treatment outside of our expertise, arrangements can be made for you to be treated in the HSE or if you wish, privately at your own cost. Please note it is possible that any private medical insurance may not cover complications arising from cosmetic work.

    Skin care products are non-returnable/refundable due to hygiene reasons.

    Hyaluronidaise treatment cannot be performed without a prior assessment and test patch.

    Liability

    Dr Sarah Kate Quinlivan will not be liable in contract, tort or otherwise for any economic loss (including, without limitation, loss of profit), or for any other special, indirect or consequential loss or damage arising out of, or in connection with, its provision of any goods and/or services to the client. It is the client’s responsibility to ensure that he or she provides us with all relevant medical details prior to each treatment. Dr Sarah Kate Quinlivan will not be liable for any damage that occurs as a result of the client’s failure to disclose such details. The client agrees to comply with all instructions and/or recommendations given to them by, or on behalf of Dr Sarah Kate Quinlivan, regarding the before and after care of a treated area.

    Complaints

    We endeavour to treat all of our patients appropriately, compassionately and fairly. If, however, in the unlikely event that you have an issue with any matter in relation to your treatment, you are entitled to make a complaint and we will be happy to provide you with our complaints policy.

    Dental Council code of practice

    The Use of Botulinum Toxin and Dermal Fillers: It is the view of the dental council that the use of botulinum toxin and dermal fillers for cosmetic treatments is not the practice of dentistry. The dental council acknowledges that the use of botulinum toxins or dermal fillers may be justified in exceptional circumstances, for example, in the treatment of temporomandibular joint disorder/dysfunction of the management of chronic pain. It is expected that in this context the treatment will be undertaken by dentists with the appropriate education, training and competence; and the treatment must be for an anticipated health gain. Only products authorised by the Irish Medicines Board can be used, even if the indication for which they are to be used is not authorised. The general conditions of use set, out in the Summary of Product Characteristics including contraindications, warnings, precautions and monitoring requirements should be complied with in so far as they are relevant to the proposed use.

    Obligations on Registered Dentists: As a registered dentist you have the following obligations :

    - if you use botulinum toxin and dermal fillers for cosmetic treatments. You must only provide these treatments if you have undertaken additional education and training, and attained the necessary competence. You must ensure that you are competent to reliably obtain written, valid and informed consent for cosmetic procedures. The patient must sign and be provided with a copy of this Code of Practice as part of the consent process. You must be aware of the conditions for the authorisation of products you use and the advice that different preparations of botulinum toxin are not interchangeable. You must have appropriate indemnity insurance cover. You must not in any way promote, market or associate these procedures with the promotion or marketing of your dental practice as a dentist. You must display a code of practice prominently in your practice and it must be situated in a place where a patient could reasonably be expected to see it before treatment.

    Information you give to us

    Medical Records – Information must be collected as part of your registration with the clinic. This includes information such as your name, address, date of birth, e-mail address, phone number, next of kin and medical history.

    Failing to provide us with this information will result in us being unable to provide treatment.

    Enquiries – We may record your details in order to track and follow-up on enquiries, in addition to scheduling appointments. This includes information such as your name, e-mail address or phone number.

    Failing to provide us with this information for marketing purposes will have no impact upon the provision of your treatment.

    Information we collect automatically

    With regards to each of your visits to our website cosmeticdoctor.ie we will automatically collect the following information:

    Technical information, including the Internet Protocol (IP) address used to connect your computer to the internet, your login information, browser type and version, time zone setting, browser plug-in types and version, operating system and platform; and

    Information about your visit, including the full Uniform Resource Locators (URL), clickstream to, through and from our Site (including date and time), products you viewed or searched for, page response times, download errors, length of visits to certain pages, page interaction information (such as scrolling, clicks and mouse-overs) and methods used to browse away from the page.

    When you provide personal information to us, the purpose for which you are providing the personal information will always be made clear.

    We take our obligations under the General Data Protection Regulation and our clinical confidentiality requirements very seriously.

    Sensitive information relating to your medical history will be kept confidential and will only be disclosed to the individuals involved with delivering your treatment.

    We may share your personal information with:

    Other members of our clinic staff as part of your ongoing care provisions, or for the purpose of analysing the business

    Selected third parties for prescriptive purposes in order to provide your agreed treatment. Opting out of sharing your information with these providers may affect our ability to treat you.

    There may also be circumstances where we are under a duty to disclose your personal information or share your personal information in order to comply with any legal or regulatory requirement, obligation or request. This includes the police for the prevention or investigation of a crime or our Insurers, legal advisors or other third parties who need access to it in the context of managing, investigating or defending claims or complaints.

    YOUR INFORMATION

    We cannot guarantee that the information you send us over the internet is secure, but once we receive it we will take all reasonable steps to protect the information you supply to us. Sk Aesthetics Limited will not transfer your data outside of the European Economic Area. Backups of electronically stored data are taken regularly, with strong encryption used to protect all files.

    Under the General Data Protection Regulation, we will only hold personal identifiable data for the maximum retention periods:

    Job applications and unsuccessful interview candidates – 6 months

    Employee Records – 6 years following termination of contract

    Medical (Aesthetic ) Records – 7 years following last appointment

    Medical (GP & Hormone) Records – Indefinitely

    Controlled Drugs Register or Prescription Information – 2 years

    Marketing Enquiries and Emails – 6 months

    Financial Records – 7 years

    COOKIES

    ACCESS TO INFORMATION

    All records will be supplied in an electronic format within 1 month of receipt of the Subject Access Request.

    All requests will be subject to the appropriate identification checks. Whilst there is no charge for the first copy of your record, we reserve the right to charge a small administrative fee for all subsequent copies of the same record.
